About the Rehabilitation in the Home (RITH) service:
RITH is an innovative service within the Aged Care, Rehabilitation and Palliative Care Division within the Northern Adelaide Local Health Network that brings hospital-level rehabilitation care directly to patients in the comfort of their own homes. Our goal is simple: to help people regain independence and achieve their rehabilitation goals without the need for an extended hospital stay.
We provide tailored programs that combine face-to-face home visits with telehealth options, offering safe, convenient, and effective care for patients recovering from illness, injury, or surgery.
Our multidisciplinary team—including Allied Health professionals, Nurses, and Rehabilitation Medicine Specialists—work collaboratively with patients and their families to design personalised rehabilitation plans. We also provide temporary equipment loans at no cost, making recovery at home easier and more accessible.
About the Role:
As a Senior Physiotherapist, you will play a pivotal role in providing advanced clinical physiotherapy services to patients transitioning from hospital to home. You'll work within a multidisciplinary team to design and implement rehabilitation programs tailored to individual goals, ensuring optimal functional outcomes.
Your responsibilities will include:
- Delivering expert physiotherapy assessment and treatment in home and virtual settings.
- Leading service development initiatives and contributing to quality improvement projects.
- Providing clinical leadership, mentoring and supervision to junior staff and students.
- Collaborating with Allied Health, Nursing, and Medical professionals to achieve patient-centred care.
- Supporting innovation through telehealth and remote monitoring technologies.
This role offers variety, autonomy, and the chance to shape rehabilitation services in a growing area of healthcare.
About You:
You are an experienced and highly skilled physiotherapist who thrives in a collaborative environment and is committed to excellence in patient care. You bring:
- Qualifications: Degree in Physiotherapy and registration with the Physiotherapy Board of Australia.
- Expertise: Extensive clinical experience in acute and rehabilitation settings, with advanced assessment and treatment skills.
- Leadership: Ability to provide guidance, supervision, and education to staff and students.
- Communication: Strong interpersonal and negotiation skills to build effective relationships with patients, families, and colleagues.
- Innovation: A proactive approach to service improvement and adaptability to new technologies like telehealth.
If you are ready to take the next step in your career and contribute to a service that empowers patients to recover at home, we'd love to hear from you.
