About us:
YAMHS is a standalone Aboriginal mental health speciality clinical in-reach and care coordination service that supports Aboriginal and Torres Strait Islander people across the lifespan. Delivered by Mob, for Mob, YAMHS operates in genuine partnership with Aboriginal communities to ensure care is culturally grounded and community-informed. The service is known for delivering culturally responsive, collaborative, and holistic mental health care, integrating clinical yarning, cultural safety, and knowledge systems. With a focus on healing and wellbeing, YAMHS provides best practice, trauma-informed mental health and social and emotional wellbeing support, underpinned by cultural expertise and clinical excellence within HNEMHS district.
YAMHS multidisciplinary team provide:
- Transitioning of Care Coordination (TOCC)- wrap around support response.
- Culturally grounded mental health assessment & care planning
- Care coordination/case management/therapeutic interventions
- Healing supports /Clinical yarning/Cultural consultation
- Collaborative partnerships with patients, consumers, families & community
- Deliver care across the lifespan as specialist Mental health clinicians.
YAMHS are breaking down the barriers for Aboriginal and Torres Strait Islander people accessing acute emergency care by creating a clinically culturally safe and welcoming space from the moment our mob arrive in ED through to mental health services and ensuring that culture and community are central to clinical care. YAMHS works in partnership with the Calvary Mater emergency department, HNE mental health services and Awabakal Wiyala medical services to help improve access to care and health outcomes and achieve health equity, to deliver culturally safe care and healing for Aboriginal and Torres Strait Islander people across the lifespan in a wraparound support response. YAMHS is committed to helping the local community access acute mental health services and work with community to empower mob to maintain their mental health and social emotional wellbeing. YAMHS also provides community-based supports & consultations outside of clinical pathways.
About the role:
This position is based at the Mater mental health centre in the YAMHS Hub and involves working closely with a dedicated team of professionals in ED and mater MH centre. The position will work in collaboration and have a specific focus on coordinating the transitions to and from Calvary emergency department to mater PECC, MHICU and mental health inpatient units /community settings for Aboriginal and Torres Strait Islander consumers. The position will emphasise care that encompass: the cultural, spiritual, physical, emotional and mental wellbeing of the individual, family or community to assist the consumer to remain connected and/or reconnect to their communities. The position can be a 7-day rotating shift roster working morning and evenings dependant on applicant. As a member of the YAMHS team you will work closely with the YAMHS clinical and SEWB workforce based in the YAMHS hub and be responsible for provision of cultural consults and support initial MH assessment with YAMHS CNC/ TOCCs and provide intensive follow-up interventions, working in a cultural trauma informed and recovery focused framework/model of care. YAMHS is looking for an experienced clinician with skills in mental health assessment, risk assessment, treatment planning and the ability to work independently as well as part of the multidisciplinary team. As the successful candidate, you will have a broad range of clinical skills and the ability to effectively manage time and prioritise tasks whilst also being engrained in Aboriginal mental health community events and building on relationship with key stakeholders, Elders and attending to men's or women's groups/business.

Our successful candidate will be passionate about improving mental health and sustaining the mental health and social and emotional wellbeing of Aboriginal and Torres Strait Islander people located in your local community. Additionally, you will enjoy working across many different mental health specialities being emergency, inpatient, community, child & adolescent, rehabilitation settings to provide culturally appropriate yarning, healing support, advocacy and consultation for our people accessing the services of Hunter New England Mental Health services.
You will be expected to be able to provide cultural knowledge and advice to our non-Aboriginal colleagues and stakeholders about local cultural protocols and the importance of incorporating holistic social and emotional wellbeing care. You will have the strength and passion to enhance Aboriginal and Torres Strait Islander consumers' engagement with Mental Health service by listening and learning from our communities lived experience. You will be dedicated to creating a culturally safe and supportive environment within the team and model self-care.
How you'll be supported:
You will be surrounded by Mob and managed by an Aboriginal service manager with a team of 35 Aboriginal staff, clinicians, undergraduates and social emotional wellbeing workforce not one day is the same. You will be encouraged and supported with self-directed learning. You will also be offered the Gayaa Dhuwi (Proud Spirit) cultural clinical supervision framework to ensure your success in this position, this framework is one of our strategies in HNE mental health service to achieve sustainable employment for Aboriginal and Torres Strait Islander people in the public health sector. As an employee you will be able to take 4 hrs a month to reconnect back to culture and maintain your spiritual wellbeing and be paid for it. You will also work alongside an Aboriginal clinical team in both child, adolescent and adult and participate in daily Aboriginal workforce safety clinical intake huddles, attend Aboriginal advisory leadership committees/forums/community events and be supported by an Aboriginal mental health workforce whilst in partnership with Wiyala Awabakal SEWB team. Requirements:
- Completed degree/qualifications in Bachelor of Health Science Mental Health (Djirruwang program), Nursing, Social Work, Occupational Therapy, or Psychology with membership of (or eligibility for) the relevant professional association and/or current registration (or eligibility for) with the Australian Health Practitioner Regulation Agency (AHPRA) in the relevant profession or employed as an Aboriginal Health Worker with eligibility for the Health Manager (State) Award Level 1, or equivalent knowledge, skills and experience. Applicant seeking appointment as a Clinical Nurse Specialist Grade 2 must also hold relevant post-registration qualifications and at least 3 years' experience working in the clinical area of their specified post-graduate qualification.
- Eligibility to drive in NSW and willingness to drive/travel as the role requires which may involve driving long distances and overnight stays.
