Our client is seeking a Part-Time Case Manager for a position located in Hopetoun. The Case Manager supports participants to achieve sustainable employment and life goals through tailored, holistic case management. This involves actively engaging participants, addressing barriers to employment, enhancing work-readiness, and connecting them with training, support services, and employment opportunities. Wednesday - Friday 35 hours per fortnight.

MAIN DUTIES/RESPONSIBILITIES


• Manage a caseload of participants with diverse needs and backgrounds;


• Conduct assessments and develop employment-focused job plans;


• Support participants through goal setting, coaching, and the development of job plans;


• Provide assistance with resume writing, interview preparation, job applications, and referrals to training and education;


• Connect participants to appropriate MEEDAC services or external providers;


• Advocate on behalf of participants with employers and the MEEDAC Employment Support Unit (ESU);


• Servicing and maintaining Centrelink and or DOT clients if applicable;


• Mentor and peer training when required;


• Deliver services in line with contractual obligations, MEEDAC policies, WHS, and quality standards;


• Maintain timely, accurate case notes and participant records using MEEDAC systems;


• Participate in outreach, activity delivery, and relief case management when required;


• Contribute to MEEDAC's continuous improvement initiatives by identifying opportunities for service enhancement;


• Ensure the safety and wellbeing of yourself, other MEEDAC personnel and your clients and customers;


• Encourage and demonstrate a positive safety culture within the company by actively promoting MEEDAC's Work Health and Safety vision and objectives;


• Contribute to, comply with, implement and maintain MEEDAC's operational work procedures and processes to comply with the

company's management system requirements;


• Contribute to, review and improve MEEDAC's Quality Management System and Work Health and Safety Management System;


• Other specific role requirements may also be stated within MEEDAC's Management Systems Documentation (e.g., procedures, policies, etc);


• Build and maintain strong working relationships with participants, team members, employers, and partner organisations;


• Provide mentoring, guidance, and support to team members to ensure consistent service quality and performance;


• Promote a collaborative, inclusive, and client-focused team environment;


• Participate in MEEDAC staff meetings, professional development, and community engagement activities;


• Complete all reporting requirements, including compliance reports, performance data, and WHS documentation in a timely and accurate manner;


• Travel as required for training, servicing, and events;


• Perform any other duties consistent with the role and organisational objectives, as directed by the Line Manager.

REQUIREMENTS


•Valid Australian Driver's Licence;


• Australian Citizenship;


• Satisfactory Australian Police Clearance and other checks as required;


• Compliance with MEEDAC and third-party requirements, including policies related to vaccination, drug and alcohol use, safety, and conduct;


• Willingness to travel as required.

SKILLS AND EXPERIENCE


• Strong written and verbal communication skills;


• Knowledge and understanding of working with culturally diverse populations;


• Proficiency in Microsoft Office applications;


• Excellent time management, organisation, and decision-making abilities;

Ability to work autonomously and in a team environment;


• Familiarity with outcome-focused or contract-driven service delivery environments;


• Culturally competent, empathetic, and professional.


• Experience in case management, employment services, or related fields;


• Capability to support participants with complex needs, including those from remote or disadvantaged backgrounds.
