The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people's homes, other community venues as well as virtual care (telehealth).
We are seeking a dedicated and compassionate Clinical Nurse Specialist (CNS2) to join our Sustaining NSW Families team — a vital early intervention and home visiting program supporting families to give their children the best start in life.
As a CNS in this program, you will provide intensive, evidence-based support to vulnerable families expecting or raising young children, promoting positive parenting, healthy child development and family wellbeing. You will build strong, trusting relationships with parents and carers, empowering them to develop the confidence and skills needed to nurture their children in a safe and supportive environment.
The Clinical Nurse Specialist position is a Registered Nurse who provides Senior Clinical Leadership in the development of specialist clinical practice and service delivery within a Nursing Unit, Team or Service. The CNS2 employs advanced clinical skills and exercises extended autonomy of decision making working closely with Managers and other Senior Clinicians to provide safe, compassionate, person-centered health care in collaboration with patients, families, carers and multidisciplinary teams.
The Sustaining NSW Families (SNF) program ideally commences during pregnancy but may commence up to 4 weeks after the birth of the infant. The program continues until the child is two years of age. Nurses utilise structured program components and assessment findings to work with families in a health promotion, prevention and early intervention approach tailored to each family's goals. This role will see you provide complex and diverse specialised Nursing Care to families and young children aged 0-2 years who are engaged with the Sustaining NSW Families program.

What you will bring to the role:
- Current registration as a Registered Nurse Division 1 with AHPRA with post registration qualification in Child and Family Health Nursing and at least 3 years' experience working in Child and Family Health, or such other qualifications or equivalent experience deemed appropriate by the public hospital or public health organisation.
- Demonstrated advanced clinical skills and contemporary knowledge of Child and Family Health Nursing practice and NSW Ministry of Health Policies and initiatives for Child and Family Health
- Demonstrated understanding of and experience working with complex and vulnerable families from 0-2 years including those from refugee backgrounds, Aboriginal, Torres Strait Islander communities and other culturally and linguistically diverse (CALD) communities.
- Demonstrated ability to work independently with minimal supervision whilst maintaining involvement within the wider multidisciplinary team and with other service providers including GPs, Community Services and NGOs.
- Demonstrated problem solving and decision-making abilities according to evidence based best practice.
- Demonstrated high level communication, liaison, negotiation and time management and report writing skills.
- Demonstrated knowledge of and ability to utilise information technology systems within a variety of applications and commitment to undertake training in electronic health record systems.
- Current Driver's Licence and willingness to use for work purpose travel.
