Southern Cross Care Queensland (SCCQ) has an exciting opportunity for a full-time Physiotherapist to join our care team at St Mary's Residential Aged Care Home in Raceview. This rewarding role focuses on delivering high-quality, person-centred physiotherapy services for our residents and Home Care clients in the Ipswich area that promote their mobility, functional independence, pain management, fall prevention and enhance overall quality of life.

About the role:

As a Physiotherapist with SCCQ, you will play a vital role in supporting the health and wellbeing of our residents and clients by providing best practice physiotherapy care within your scope of practice, including but not limited to the following:

  • Providing skilled, holistic mobility, strengthening and falls prevention assessments, consultation, history taking, diagnosis and treatment services.
  • Supporting face to face consultations with remote telehealth reviews as required for remote referrals.
  • Prescribing and implementing physical responsive and preventative assessments to enhance resident and client mobility needs as referred and following up on appropriate course of action.
  • Following up with residents appropriately to ensure positive health outcomes, monitoring of progress and review of care as care needs change.
  • Referring residents appropriately to other providers if their needs exceed the PT scope of practice.
  • Care coordination with General Practitioners and other Allied health professionals to provide optimal care with strong focus on building strong collaborative relationships built on foundation of trust and professional collegial respect.
  • Documenting all care provided and education/information given to patients within their health record, as per professional and company stands within the electronic record.
  • Supporting residents and clients in fulfilling their choices and preferences.
  • Participating in SCCQ Clinical Governance and Continuous Quality Improvement objectives in order to maintain and support good clinical practice

What will you bring to the role?

  • Bachelor degree and AHPRA registration as Physiotherapist
  • Strong clinical reasoning. assessment and decision-making skills
  • A broad scope of practice, including but not limited to diagnosis of acute mobility, dexterity, pain and rehabilitation conditions in older people (desired)
  • Experience in aged care, rehabilitation or community health (desirable)
  • Commitment to evidence-based practice and continuous quality improvement
  • Excellent communication and interpersonal skills with a strong sense of empathy and respect for older people
  • Ability to work collaboratively in a multidisciplinary team
  • Professional Liability Insurance as legally required for all practicing PTs.
