About the Role
We are seeking a compassionate and proactive Client Onboarding Specialist - Aged Care (internal job title is "Concierge") to join our team and serve as the first point of contact for prospective Aged Care, homecare support clients and their families. This vital role focuses on understanding individual needs, building meaningful relationships, and helping clients navigate their entry into Anglicare Southern Queensland's community and home-based services.
As a Concierge, you'll help new clients begin their journey with Anglicare by identifying support needs, facilitating service agreements, providing Anglicare's Home Care Packing programs and ensuring timely and appropriate service delivery. You'll use a people service approach to connect clients with the right services, offering clear guidance and seamless onboarding.
This is a full-time, permanent position based at our Eight Mile Plains office. The role is flexible, autonomous and follows a hybrid working model, with one day per week in the office and the remaining days spent working from home or conducting remote client consultations.
Responsibilities
- Act as the first significant point of contact for potential clients, carers, and families
- Conduct client meetings to identify goals, preferences, and service needs
- Provide clear and accurate information about Anglicare's services and support models
- Establish and maintain Service Agreements, ensuring all required documentation is completed
- Initiate and coordinate the setup of services, liaising with internal teams for scheduling and staff matching
- Monitor early service delivery to ensure alignment with client expectations and needs
- Maintain accurate and timely records in client systems, including AlayaCare
- Collaborate with internal teams to connect clients to appropriate services and supports
- Promote Anglicare's services through community engagement and relationship building
- Participate in continuous improvement activities and support a strong team culture
- Ensure compliance with Anglicare's policies, procedures, and person-centred care standards
About You
You're a confident communicator with a genuine passion for helping others. Whether you're speaking with a new client, their family, or a community partner, you approach each conversation with empathy and clarity. You thrive in a fast paced environment and are comfortable managing competing priorities while maintaining strong attention to detail.
You bring a solution focused mindset and can confidently guide others through sometimes complex systems, helping them feel informed and supported. You enjoy being part of a team but are also self-driven, highly organised in your approach to work and trustworthy to work autonomously.
Requirements
- Diploma in Social Sciences, Behavioural Sciences, Health, Human Services or a related field (desirable)
- National Police Certificate (current or willingness to obtain)
- Current Queensland Driver's Licence and ability to travel within the region
- Experience working with Sugar CRM and AlayaCare (desirable)
- Experienced using Microsoft Office suites, particularly confident and skilled in Excel
- Proven experience in client acquisition, customer service and sales
- Exceptional interpersonal, written, and verbal communication skills
- Knowledge or understanding of the aged care, home care and/or disability sectors (highly regarded)
- Ability to manage multiple tasks and meet deadlines with minimal supervision
- Comfortable using a range of software applications and open to learning new systems
- Commitment to confidentiality and professional integrity
