1. Role Purpose
The LOTO / PTW Supervisor establishes, administers, and continuously improves the site's Lock‑Out Tag‑Out (LOTO) and Permit to Work (PTW) systems to eliminate or minimise risk from hazardous energy and high‑risk work. The role provides day‑to‑day coordination of all permits (electrical and non‑electrical), ensures competency and compliance, manages SIMOPS conflicts, and acts as the single point of truth for work authorisation and boundary control across construction, commissioning, operations and maintenance.
2. Key Accountabilities
PTW System Ownership & Governance
Maintain the site PTW standard and procedures (hot work, confined space, excavation, working at heights, lifting, LV electrical, HV access interface, live testing exceptions).
Ensure permits reflect current legislation, codes, and company standards; lead periodic reviews and updates.
Daily Permit Coordination (SIMOPS Control)
Run the daily PTW meeting/board; issue, sequence, and coordinate all permits; manage hold points and interdependencies.
Perform SIMOPS and conflict checks (space, time, energy, plant movements); enforce controls (barriers, isolations, spotters, exclusion zones).
Maintain the live PTW schedule, area maps, isolations, and boundary controls.
LOTO Administration & Assurance
Oversee isolation planning and LOTO integrity for LV; interface to HV Operating Authority for HV isolations and permits.
Control lock/hasp/lockbox/tag registers; allocate IDs; manage key custody; administer lost‑key and break‑glass protocols.
Verify absence of voltage/isolation testing records and compliance with IC (Isolation Certificate) requirements.
Competency, Training & Authorisation
Maintain competency matrices and currency (First Aid/CPR/LVR for LV isolators; HV switching authorisations for interface roles).
Deliver PTW/LOTO inductions and refreshers for Permit Applicants, Holders, Users; coach supervisors on quality permits and SWMS/JSEA integration.
Records & Digital Systems
Own the PTW Register, Isolation Certificate Register, LOTO Register, DNOT/earthing logs (where interfacing with HV), calibration records for test instruments.
Ensure records are complete, accurate, retrievable; support auditing and client/TNSP reviews.
Audit, Inspection & Continuous Improvement
Conduct planned and spot audits of permits in the field; verify controls (barriers, gas tests, AoV tests, rescue gear).
Track findings, actions, and trends; drive procedural improvements; report KPIs.
Commissioning & Energisation Support
Define construction/commissioning boundaries (administrative controls), handover conditions, and NOE (Notice of Energisation) workflow.
Coordinate with Engineering/Commissioning on energisation plans, lock plans, and staged boundary moves.
Incident Response & Investigation
- Act as SME for hazardous energy and permit breaches; support emergency response; participate in investigations and corrective actions.
3. Decision Rights & Delegations
Authority to approve, place on hold, or revoke permits where controls are inadequate.
Authority to stop work for non‑conformance to PTW/LOTO or SIMOPS controls.
Authority over lock/hasp/lockbox/tag issuance and ID allocation; escalate missing/compromised devices.
Escalation to Operations/HSE Manager and HV Operating Authority for boundary changes, switching impacts, or deviations.
4. Qualifications, Licences & Training
Certificate III Electrical trade or HSE qualification (or equivalent experience).
Desired: Australian electrical licence (for LV isolation governance) or demonstrated LV isolation competence; HV awareness training.
PTW/LOTO training to supervisor level; incident investigation (ICAM or similar) desirable.
Current First Aid (HLTAID011) and CPR (HLTAID009); LVR (for LV oversight) preferred.
Construction White Card; driver licence.
5. Knowledge (Australia)
WHS Act & Regulations; QLD Electrical Safety Act & Regulation; Managing Electrical Risks in the Workplace Code of Practice.
AS/NZS 4836 (LV safe work), AS/NZS 3000 (Wiring Rules), AS 2865 (Confined Spaces), AS 1735/2550 (lifting/plant), AS 1319 (signs).
ENA NENS 01/03 safe access frameworks; HV Operating Authority interfaces and exclusion zones.
Client/network PTW systems and TNSP/DNSP outage and switching coordination requirements.
6. Experience & Skills
5+ years in industrial/construction/renewables/resources with PTW and LOTO responsibility.
Proven SIMOPS coordination on multi‑contractor sites; strong field presence.
Isolation planning (electrical & non‑electrical); quality SWMS/JSEA integration with permits.
Excellent communication, coaching, and conflict‑resolution skills; disciplined recordkeeping.
7. Detailed Responsibilities
7.1 Permit Lifecycle
Application: check scope, drawings, competence, gas tests/rescue plans where applicable.
Authorisation/Issue: verify isolation lists, boundaries, monitoring, and emergency arrangements; brief Permit Holder and Users.
In‑Progress Control: conduct field verifications; manage changes (suspension/re‑issue for scope changes).
Close‑out: verify area reinstatement, tool/people clearance, documentation complete; update registers.
7.2 LOTO & Isolation Integrity (LV focus)
Ensure isolations achieve a visible break where practicable; verify remote/auto disabled; confirm AoV testing performed and recorded.
Manage group lock box use; ensure every worker applies/removes a personal danger lock/tag.
Oversee alternate energy isolation (mechanical/hydraulic/pneumatic/gravitational/stored energy).
Interface to HV OA for DNOTs, operator/working earths, and HV permit conditions impacting LV works.
7.3 Commissioning Boundaries & NOE
Maintain construction vs commissioning boundary lock plans; control "Commissioning in Progress" administrative tags/locks.
Administer Notice of Energisation approvals and communication before boundary moves/energisation.
7.4 Tools & Systems
Administer digital PTW platform (or registers); ensure daily status boards and area maps are current.
Maintain calibration status of gas detectors and electrical test instruments used for permit pre‑conditions.
8. KPIs
Zero PTW/LOTO related incidents or uncontrolled energisations.
100% permits audited per plan; 100% mandatory fields completed.
100% competency currency for Permit Holders/Isolators/Users.
Daily SIMOPS/permit board maintained with documented conflict checks; zero SIMOPS‑related events.
Close‑out documentation within 24 hours of work completion.
9. Key Interfaces
- Operations, Electrical, Commissioning, Construction, HSE, Contractors/Subcontractors, HV Operating Authority, TNSP/DNSP, Client representatives.
10. Physical & Other Requirements
Fit for substation/industrial environments; capable of wearing arc‑rated PPE and respiratory protection if required.
After‑hours availability for critical works; travel to satellite sites.
