Where you'll be working

The Transition to Residential Aged Care Unit (TRAC) is a unique 20-bed ward where acute care nursing meets residential aged care. Our patients have recovered from an acute medical episode but are no longer safe to return home. Here, we partner closely with families and carers to support patients as they transition into an Aged Care facility. It's a service where compassionate nursing meets one of life's biggest turning points.

About the role

As a TRAC nurse, you'll provide geriatric clinical care, palliative support, and essential social and emotional care to patients and their loved ones. Every day brings meaningful connection as you guide families through what can be a difficult and deeply personal journey. This is more than nursing – it's about being present, offering reassurance, and helping patients and families navigate change with dignity and compassion.
