Assessment and Planning Officer - Carer Gateway

Location: Preston

Duration: 8-week locum

Hours: Full-time or 0.8 FTE

About the Role

You'll conduct holistic assessments and develop tailored action plans for carers, ensuring they receive appropriate referrals and supports. This is a fast-paced, rewarding role where your interpersonal and organisational skills will make a real difference.

Key Responsibilities

* Undertake Carer Star assessments and develop individualised action plans

* Manage referrals, coordinate supports, and maintain accurate documentation in DC2

* Work collaboratively with internal and external stakeholders to ensure positive outcomes for carers

About You

* Previous experience in assessment or case management within community, health, or aged care sectors

* Familiarity with Carer Star and DC2 systems highly regarded

* Strong communication, empathy, and problem-solving skills

* Available to commence immediately
