The Stroke Foundation is a national not-for-profit organisation that works with stroke survivors, carers, health professionals, government and the public to reduce the impact of stroke on the Australian community. Our mission is to Prevent Stroke, Save Lives, Enhance Recovery.

Purpose of the team and position

StrokeLine Officers are part of the Navigation and Access team, which is part of the Stroke Programs, Research and Innovation division. StrokeLine Officers report to the StrokeLine Manager.

This role contributes to our vision of fewer strokes, better outcomes, and support and care for all. This role:

  • Empowers people to reduce their risk of stroke, and to recognise the signs so they can access treatment quickly.
  • Ensures survivors of stroke, and their families and carers have what they need to manage their health, achieve their best possible recovery and live a good life.

This position delivers StrokeLine services. These are:

StrokeLine inbound. Responding to requests for service via telephone, email and online. StrokeLine officers deliver information, advice, support and referral to survivors of stroke, carers, family members and to the public. Services range from brief interventions to short term assistance with service system navigation.

StrokeLine Navigator. Reaching out to survivors of stroke and their families after they leave hospital. Services are delivered by telephone, and offer information, advice, support and referral, and if needed, short-term assistance with service system navigation.

Living Well After Stroke. An 8-week group-based video conferencing program for survivors of stroke. Living Well After Stroke empowers survivors of stroke and transient ischaemic attack to change health behaviours and reduce their risk of another stroke.

Key responsibilities

  • Deliver information, advice, support and referral via phone, email, video conferencing and online.
  • Assess people's needs, screen for risk and complexity, and make a plan to meet those needs, following up as required.
  • In the role of daily lead, prioritise and manage the client queue, allocate clients to team members, supervise calls and client records, and manage handovers.
  • Supervise StrokeLine administration volunteers.
  • Work with the StrokeLine Manager and the StrokeLine team on service planning, organising, monitoring, reporting and improvement.
  • Develop positive and effective relationships with staff and stakeholders.
  • Provide expert advice on the information and support needs and preferences of survivors of stroke, families and carers, and the general public.
  • Contribute as needed to Navigation and Access team projects to improve our products and services.
  • Support the involvement of survivors of stroke, families and carers across the Stroke Foundation, as a key part of our mission.

Your background, skills, and experience

Qualifications (Essential)

  • Tertiary qualification in nursing or allied health disciplines of social work, clinical psychology, speech pathology, physiotherapy, occupational therapy or dietetics.

Skills

  • Engagement, assessment and intervention skills.
  • Building positive and effective relationships with staff and stakeholders.

Knowledge and experience

  • Strong knowledge of stroke and the Australian health care and community service system.
  • Delivering information, advice, support and referral to people with health conditions, their families and carers, and to the general public.
  • Delivering services to people with complex needs, who are vulnerable or at risk.
  • Delivering services over the phone, via email and online.
  • Identifying opportunities, changes, concerns and risks and ensuring appropriate responses.
  • Lived experience of stroke, illness, disability or caring (desirable).
