About the role
An opportunity to join a multidisciplinary Community Mental Health Team providing recovery focused care to consumers experiencing mental illness. A unique opportunity to be a part of a ground breaking Brief Intervention Team offering short term support to clients referred to the service who do not require longer term case management. work within a welcoming, friendly and highly skilled team at the Wyndham Clinic, Hoppers Crossing. We are passionate about providing an accessible high-quality service to the rapid growing, diverse community we serve.
ABOUT YOU:
Qualification - Social Worker Grade 2
Eligible for Social Work Membership Endorsement.
Excellent communication and documentation skills
Ability to work in a multidisciplinary team.
The ability to think critically and problem solve effectively
Be consumer recovery focused care with consumers, inclusive of carers / families.
A good working knowledge of the Victorian Mental Health Act and relevant legislation
Primary Purpose/Focus of the role
SE in the Brief Intervention Role provides short term support to clients aged 25-65yrs
Provide quality mental health and discipline specific, client centered assessment and treatment that is evidence based.
Provide linkages and referral pathways for clients requiring short and long term support
Offer consultation to the team and facilitate service linkages.
Report writing and liaison with GPs in the Community when required.
Effective and efficient service linkages within the community.

A current Police Record Check and Working with Children Check.
