- Heart Failure Nurse Practitioner - 2 days per week

About Cardiology
The Department of Cardiology at Austin Health is committed to providing high quality and compassionate care to patients with all forms of heart disease. It also provides a consultative service for other units within Austin Health. The department has a strong clinical, teaching and research focus.
Austin provides a comprehensive multidisciplinary Heart Failure Service that supports patients in a range of settings including in the community and whilst an inpatient.
About the role:
The Heart Failure Nurse Practitioner role aims to optimise patient outcomes during the acute phase of illness in ED and inpatient setting and during the recovery/transitional period in the outpatient clinic and via telehealth.
The Heart Failure Nurse Practitioner (NP) is directly involved in the provision of expert patient-centred clinical care and is recognised as a senior clinical nurse in their area of specialty. This extends to the care of family, carers, and other health professionals.
A regular day will see you:
- Providing high quality care to patients with Heart Failure in both the inpatients and outpatient settings
- Work collaboratively with clinicians within the admitting unit and GPs, supported by the Director of Heart Failure Services & Cardiology to improve systems of care
About you
You will be someone who has excellent communication skills, with a strong commitment to excellence in patient care. You'll bring energy and drive to your work, and have a keen interest in developing your skillset.
To be successful in this role, you'll bring with you:
- Extensive experience in the management of heart failure
- Experience in developing and implementing nurse led models of care, preferably in a tertiary setting
- Nursing & Midwifery Board of Australia endorsement as a Nurse Practitioner
- Post graduate certificate/diploma in critical care nursing
