Duties
It's an exciting time at icare where no two days are the same; as a Team Leader you will:
- Lead a team to ensure person centred, timely, high quality and cost-effective services are provided to meet participant's/workers needs and scheme objectives.
- Partner with the Head of Lifetime/Workers Care to roll out the new service delivery model, and drive the capability uplift across the team to enable a significant uplift in the experience for those we serve.
- Lead the embedding of the complexity framework, that enables segmenting our participants and better allocation of case loads across the team.
- Coach and monitor team performance, providing regular feedback and supporting Case Managers with the onboarding of participants/workers as they enter the scheme.
- Lead a team to achieve business outcomes that are aligned to the KPI's that monitor and measure key areas of the role. This includes, but is not limited to workforce planning individual and team workload management, and onboarding new team members
- In collaboration with the Head of role, make decisions for the reasonable and necessary treatment and care of people with spinal cord injuries and severe brain injury consistent with legislative requirements and established guidelines to maximize participant choice, quality of life and community participation, whilst ensuring the long-term viability of the Scheme.
- Work collaboratively as a member of the regional leadership team by ensuring a consistent delivery of service and facilitating organisational change in alignment with regional initiatives.
- Coordinate the establishment of support by the relevant team member for new participants/workers on a case by case basis. This includes nurturing relationships with participants/workers and their family during their application to the scheme by; providing information/support about the processes and expectations of the scheme, linking families to appropriate community and mainstream supports, building strong collaborative relationships with hospitals and referral partners and ensuring a smooth transition to an appropriate support person.
- Foster a team culture to drive high performance and support the integration of roles within the team to ensure collaborative, evidence based decision-making practices are applied to deliver optimal outcomes to participants/workers and their families, ensuring adherence to protocols.
- Contribute to service improvement strategies and a learning culture by actively collaborating across Care and Community in the design and implementation of internal quality assurance, and continuous improvement systems to drive quality and consistency in decision making.
- Demonstrate, support and facilitate critical reasoning and thinking across a range of complex and/or time sensitive decision-making environments involving participants/workers, internal and external stakeholders in collaboration with internal specialist services and utilisation of resources.
- Manage and resolve frontline complaints or contribute to the resolution of escalated complaints and issues raised by stakeholders.
- Identify and participate in own professional development opportunities to ensure currency of skills and knowledge regarding current evidence and best practice.
Skills & Experience
- Relevant tertiary qualifications in a health or disability discipline
- Pre-Employment Checks
- Experience in leading a team and coordinating multiple stakeholders dealing with people and families experiencing serious illness or injuries or disability.
- Critical reasoning skills to ensure a high degree of integrity in the decision-making processes.
- Ability to set clear expectations and engage in difficult conversations in a simple and empathetic manner.
- A current driver's licence, with the ability to travel.
- Up to date with current Public Health Order requirements, industry trends, best practices and regulatory changes
- Maintain a current Working with Children Check
- A broad range of experience leading a team in coordinating multiple stakeholders supporting people and their families experiencing serious injuries and or significant and complex disabilities.
- Established community networks in the health/disability sector.
- Experience working with people with brain and/or spinal cord injuries.
- Experience and understanding of Workers Compensation insurance.
