ABOUT THE ROLE

Due to our expansion, we have an exciting opportunity for a motivated, self-directed and experienced clinician to join our team as Care Manager, reporting directly into a Regional Care Manager.

  • A Registered Nurse by profession, the Care Manager is a key role responsible for the oversight of care and services to clients
  • This rewarding and hands on role requires a proactive approach to the planning, coordination, delivery, and care management of clients and services.
  • Working closely with the Rostering Team, Client Liaison Team and the role includes engaging with and leading our care team, consisting of Nurses and Carers.
  • Full-time positions available in our South East region (Toorak, South Yarra), East region and Mornington Peninsula.

We are looking for people who have:

  • A current registration with AHPRA as either a Division 1 Registered Nurse
  • Registered Nurses with at least 5 years experience in the field.
  • Knowledgeable in Support at Home and/or Home Care Packages (strongly regarded)
  • A passion for caring for people and delivering a high standard of care.
  • Excellent communication, customer service and interpersonal skills.
  • Exceptional organisational and problem-solving skills.
  • Access to a smartphone.
  • A current driver's license and access to a registered, insured and well-presented car.
  • The right to work in Australia (Mandatory)
  • NDIS Worker Screening Check (Mandatory)
