About the Role:

Are you passionate about leading with purpose in a values-driven organisation? At Uniting, we are seeking a committed and proactive Team Leader to support the day-to-day operations of Home and Community Care Service. In this role, you'll lead a team of support workers and clinicians in conjunction with the broader team, ensuring high-quality, person-centred care while fostering a positive and respectful environment for clients and staff alike.

You will work collaboratively with the Home and Community Care team to ensure smooth service delivery, compliance with care standards, and staff development, while championing Uniting's mission of compassion and inclusion.

Your Key Accountabilities:

• Support and supervise a team of support workers and clinicians
• Support the delivery of safe, respectful, and person-centred care to clients.
• Support effective rostering, team performance, and supervision of delivery of services.
• Ensure compliance with relevant standards, policies, and procedures.
• Model and promote a positive workplace culture and professional practice.
• Build strong relationships with clients, families, staff, and health professionals.
• Contribute to continuous improvement initiatives and service planning.
• Support staff onboarding, development, and performance management processes.

Your Qualifications and Experience:

• Certificate IV in a relevant field (such as Business, Aged Care or Health or equivalent) or willingness to complete. Even better Bachelor qualifications in nursing or allied health and current registration with the Australian Health Professional Registration Authority (AHPRA) or other relevant
• Minimum 2 years' experience in aged care, ideally in a leadership or supervisory role.
• Strong understanding of person-centred care and aged care compliance standards.
• Experience supporting teams, mentoring staff, and managing rosters.
• Excellent interpersonal and communication skills.
