About the Role:
Permanent, full-time position providing crisis assessments, interventions, counselling, and psychoeducation, while coordinating access to psychosocial supports and community services tailored to individual needs in acute care services.
Proactively monitor and respond to changes in consumer risk status, ensuring timely, clinically appropriate escalation and intervention.
Foster strong collaboration with internal teams and external partners — including health professionals, crisis accommodation services, and the Homeless Health Assertive Outreach team — to ensure seamless, coordinated care.
Champion consumer advocacy by addressing personal needs, concerns, and preferences, in full compliance with the NSW Mental Health Act (2007; amended 2015).
Support a culture of learning and improvement by contributing to staff development, delivering training within the multidisciplinary team, and participating in ongoing service enhancement initiatives.
About You:
Degree in Social Work, eligible for membership of the Australian Association of Social Workers (AASW) and a Class C driver's licence.
Experience in community mental health, with confidence in acute and triage settings, risk assessment, and crisis intervention.
Committed to holistic, person-centred practice that includes families and carers as part of the care team.
A natural collaborator who builds strong, purposeful partnerships across services to deliver seamless, compassionate wraparound support for complex caseloads
You're highly organised, with excellent communication skills and confidence using electronic medical records to manage complex caseloads.
