Aboriginal Family Support Services (AFSS) is a lead, Aboriginal Community Controlled Organisation that has been providing services to Aboriginal families and communities across South Australia for over 40 years.

The Aged Care Connector (ACC) is responsible for supporting Aboriginal and Torres Strait Islander clients in navigating access to local aged care support services. The primary objective is to provide advocacy support, ensuring clients receive culturally safe plan assessments. The ACC will guide clients in understanding their aged care service entitlements, including My Aged Care registration, contractual arrangements, and costs. Additionally, they will assist Elders and their families in being well-prepared for plan reviews and outcomes. The role involves close collaboration with aged care Assessors to ensure Elders receive comprehensive support throughout the assessment process. Emphasizing cultural safety and trauma-informed practice, the ACC will work closely with the Aged Care Connector to address the specific needs of each client.

Who we are looking for

  • Oversee the development and maintenance of networks with the community services sector (including Aboriginal agencies, government services and those provided by mainstream NGO and not for profit sectors)
  • Communicate in a culturally responsive manner with Aboriginal individuals, groups, families and organisations, provides support to enable client access to appointments, and record all direct and indirect support in a manner that shows a respect for the time used and the utilisation of an aged care package.
  • Provide understanding of the principles of person-centered care and trauma- informed practice and active implementation AFSS practice model of case work for working with individuals, groups and families.
  • Adhere to the Government of South Australia's Information Sharing Guidelines, which are designed to give providers of services to children, young people and adults, confidence in sharing information to prevent harm or respond to current threats to safety and wellbeing.
  • Maintain and model an ongoing commitment to continuous improvement in the provision of services to internal and external customers.

To be successful you will need

  • A South Australian Working with Children Check (WWCC)
  • National Police Clearance
  • Safe Environments for Children and Young People Certificate
  • NDIS Worker Screening Check clearance
  • Valid and full South Australian Driver's Licence
  • Unrestricted eligibility to work in Australia
  • Vulnerable Persons Clearance.
  • A qualification in health or care related field and Current First Aid.
