We understand that support looks different for everyone and can make an incredible difference, that's why Autism Queensland takes tailored evidence-based approaches to every client. Our Lifestyle Support Workers (LSW's) walk alongside our clients to build capacity, support independence and empower a life of participation, opportunity, and choice. Taking a strengths-focused approach to person-centered care means that our LSW's are creating a safe, healthy and inclusive environment where clients can engage, on their own terms.
Equipped with skills like adaptability, kindness and patience, this role will provide support to our clients through;
- Providing healthy and hygienic home environment for adults in Independent Living Service Houses, ensuring required equipment and consumables are available and ready for use.
- Utilising our Active Support Framework to support clients as detailed within their support plan in managing activities of daily living which may include high intensity daily personal activities.
- Developing and maintaining positive, respectful relationships with clients and support clients in the development of positive and valid community relationships.
- Ensuring all support provided is done so in alignment with NDIS Practice Standards managing any associated risks and reporting all incidents.
- Assisting in the development and implementation of positive behaviour support plans to manage and monitor complex and challenging behaviours.
- Working collaboratively as part of a multi-disciplinary team to provide a holistic approach to support.
- Transporting clients safely as required to attend day programs, medical appointments or for community access.
- Maintaining appropriate documentation for each client.
With the complex care requirements of this role, we are searching for a candidate that comes equipped with;
- Certificate III in Disability or Individual Support (or relevant qualification)
- The ability to work in various locations across Brisbane, Logan and Ipswich.
- Strong understanding of various components of an individual support role, which could include positive behaviour support plans, medication and community access.
Please note that this is not an entry level role, we are looking for someone with at least 6 months of experience working within a support role.
