- SCHADS Level 4 | $44–$48 per hour + super

As a Case Worker at Freeman House, you will support people on their recovery journey through tailored case management, one-to-one support and group work. Your focus will be on improving health, wellbeing and social outcomes, while empowering clients to achieve long-term stability and connection within the community.
Your ResponsibilityProvide case management and treatment support in line with best practice and the Society's AOD Model of Care
Support clients transitioning from withdrawal to residential rehabilitation and community-based services
Facilitate group therapy sessions and deliver individual psycho-social support
Work collaboratively with a multidisciplinary team to achieve positive client outcomes
Maintain accurate client records and contribute to service improvement
Engage with families and significant others when appropriate
Conduct psychosocial progress assessments and participate in case reviews
This is an identified role under Section 14d of the Anti-Discrimination Act 1977 and as such Aboriginality is an essential requirement of the role. is of Aboriginal and/or Torres Strait Islander descent, and
identifies as an Aboriginal and/or Torres Strait Islander person, and

Experience managing a caseload and supporting individuals with complex needs (AOD, homelessness, mental health, or justice system involvement)
Diploma in AOD or a related community services field with relevant experience, or a degree in Social Work, Counselling, or another related discipline
Ability to deliver evidence-based treatment approaches to support recovery
Strong communication skills and ability to work effectively within a team
Commitment to ethical, professional, and culturally sensitive practice
A valid NSW driver licence
