Hospital in Your Home (HiYH) provides hospital-level care in the community, supporting hospital avoidance, early supported discharge, and community-based rehabilitation. We are seeking an experienced Community Physiotherapist to deliver physiotherapy services under our GEMITH (Geriatric Evaluation and Management in the Home) and Frailty models of care.

This role is ideal for a physiotherapist with strong experience in community physiotherapy, home-based rehabilitation, and working with older adults and complex patients.

About the Role

You will provide physiotherapy assessment and treatment in patients' homes and community settings, supporting functional recovery, mobility, balance, strength, and falls prevention. You will work autonomously while being embedded in a multidisciplinary community health team, including nurses, nurse practitioners, allied health assistants, occupational therapists, geriatricians, and other allied health disciplines.

Key Responsibilities
  • Deliver community physiotherapy assessments and treatment in home-based settings
  • Provide community-based rehabilitation and reablement programs
  • Support hospital avoidance and early supported discharge pathways
  • Manage patients with frailty, deconditioning, chronic disease, and post-acute needs
  • Participate in multidisciplinary care planning and case conferences
  • Identify and escalate clinical deterioration in line with clinical governance frameworks
  • Provide falls prevention, mobility training, and home exercise programs
  • Complete timely clinical documentation using digital health systems
Essential Criteria
  • Bachelor or Master of Physiotherapy
  • Current AHPRA registration
  • Demonstrated experience in community physiotherapy and community-based rehabilitation (essential)
  • 3+ years' experience as a practitioner
  • Experience working with older adults, frailty, sub-acute, or rehabilitation patients
  • Ability to work independently in community and home-based environments
  • Strong communication, time management, and clinical reasoning skills
  • Current driver's licence and access to a vehicle
Desirable
  • Experience in GEMITH, Hospital in the Home, transitional care, or sub-acute rehabilitation
  • Experience in multidisciplinary community health services
  • Interest in innovative models of care and delivering care closer to home
