About the role
The Lifestyle Coordinator is responsible for identifying and responding to both the individual and group needs of the home's residents to enhance the quality of life and to promote social, physical, psychological, spiritual, cultural independence and interaction, both inside and outside of the home, whilst maintaining and supporting the residents' choice.
Duties
- Lifestyle assessment, intervention and evaluation
- Documentation - Care plan development and review
- Work with the wider nursing / allied health team to contribute to resident care and emotional wellbeing
- Deliver group and individual recreational activities/programs to residents.
About You
- 3+ years' experience as a Lifestyle / Activities Coordinator in an aged care facility
- Compassionate and enjoys working in partnership with the nursing team, residents and their families
- A recognised qualification in Leisure and Health, ideally Cert IV in Leisure and Health
- Friendly with a positive attitude and thrives working as a team
- Ability to work independently and lead a team to deliver care
- Confident in assessments and updating care plans
- Exceptionally organised
