About the Role

Centrally located and close to multiple public transport routes. A great opportunity to increase your skills in a senior capacity. You will provide a high standard of clinical assessment, treatment and support to clients of the St. Vincent's integrated adult MHS and their carers, demonstrating excellence in discipline specific and recovery-focused case management skills.

You will join a dynamic community mental health team operating under a recovery-oriented model of care, utilising the Strengths Model of Case Management.

  • Work within the Strengths Model of recovery orientated practice providing supervision and leadership to staff
  • Work one to one and build a therapeutic alliance with the consumer and family/carers actively engaging them in the planning, delivery and evaluation of their care
  • Undertake the assessment, provision of treatment and delivery of targeted interventions to consumers with complex mental health related needs and their family/carers

Classifications for this position will range from RPN Grade 4 NP75 - NP77 ($112,465 - $121,113 per annum pro rata)

Your Contribution

  • Current registration with AHPRA
  • A minimum of 5 years post qualification clinical experience for Nursing
  • Extensive experience in the provision of assessment, discipline specific intervention and case management services for consumers experiencing early psychosis, as well as providing appropriate support for their carers and families within a recovery framework
  • Experience in providing clinical supervision to other mental health staff
  • A comprehensive understanding and working knowledge of the Mental Health Act 2022 (including amendments) and other relevant legislation
  • Current Victorian driver's license
  • A valid Nationally Coordinated Criminal History Check, or willing to obtain

LOCAL WORK ENVIRONMENT

St. Vincent's Mental Health (SVMH) includes an adult area mental health service for the inner city areas of Yarra and Boroondara.  The adult mental health service components are: The Footbridge Community Care Unit (CCU), the 44 bed Acute Inpatient Service (AIS) located on the St. Vincent's Hospital site, the North Fitzroy PARC and two community mental health services (CMHS) at Hawthorn and East Melbourne (Clarendon CMHS). Community mental health services comprise the Triage and Crisis assessment and Treatment (CAT) Service and the Primary Intervention Care Team (PICT) includes Extended Triage and HOPE (Suicide Prevention),   Continuing Care (CCT), Mobile Support and Treatment (MST) and Homeless Outreach (CHOPS) services.  A Consultation & Liaison Service is provided to the health service. NEXUS Dual Diagnosis Service, the Body Image and Eating Disorders Assessment and Treatment Service (BETRS) are regional services. Statewide services are the Victorian Dual Disability Service and the Victorian Transcultural Mental Health. The Acute Inpatient Service includes 5 beds designated as the Koori State-wide Inpatient Service and linked to the Victorian Aboriginal Health Service.

St Vincent's Mental Health is committed the recruitment and retention of high quality staff.  The service aims to support staff to maintain and develop their skills to provide excellent clinical care and to meet their own personal and career goals.  SVMH has a strong education and research focus in addition to affiliations with a number of universities and educational institutions.  All SVMH clinical staff are expected to be familiar, and practice in a manner consistent with the National Practice Standards for the Mental Health Workforce.  

POSITION PURPOSE

The Clarendon Continuing Care Team provides ongoing specialist treatment and support to meet the identified needs of clients who suffer with a serious mental illness and associated high level of disability in the City of Yarra.  Intervention includes a range of clinical and therapeutic interventions using the 'Strengths' model of individual case management. 

Under the guidance of the Team Manager and Team Consultant Psychiatrist, and in conjunction with team members provide a high standard of clinical treatment and support for a case load of clients presenting with multiple complex problems including serious mental illness, associated social relationship issues, everyday living skill needs and Dual Diagnosis.  The position also provides leadership and supervision functions for nursing and more junior staff, actively contributing to quality activities at a local and mental health service level. In addition to the clinical work a significant component of the role involves the development and maintenance of sustainable working partnerships with a range of other service providers both internal and external of the St Vincent's' Health organisation.

St Vincent's Mental Health utilises the 'Strength's' model of case management, the position will actively contribute to the implementation of the Strengths model at the Clarendon site and across the wider mental health service.  

POSITION DUTIES
  • Model a high standard of clinical practice to multidisciplinary staff members, and actively contribute to the development of the clinical practice within the mental health service as a senior staff member.
  • Provide clinical services to consumers and carers, including the provision of high-quality mental health assessments, case management, duty function, code grey response and coordination, and clinical interventions in line with SVHM Policies and Procedures.
  • Build a therapeutic alliance with the consumer and family/carers and actively engage them in the planning, delivery and evaluation of their care.
  • Develop responsive and accessible services and continuity of care through effective links and liaison with the integrated adult area mental health service and other relevant services.
  • Provide case management to cover secondary caseloads for other case managers when required. 
  • In liason with other senior staff provide effective leadership. This will include allocation of portfolios related to discipline responsibilities and areas of interest.
  • Participate in regular line management with Manager and/or delegate
  • Contribute to the development of policies and procedures for the service.
  • Actively participate in the supervision, education, and mentoring of non-senior staff within the St. Vincent's Mental Health Program
  • Meet expected key performance indicators (KPIs) and maintain all documentation in line with SVMH policies.
  • Support the team manager, where required, to help supervise, lead, and oversee daily operations of the clinic or perform other delegated duties
SELECTION CRITERIA

ESSENTIAL REGISTRATION, LICENSE OR QUALIFICATION REQUIREMENTS

Formal education/ qualifications:

  • Bachelor Degree in Occupational Therapy or Social Work, Clinical Masters in Psychology and current Registration as a Psychologist under the Health Professions Registration Act 2005 (Vic), Division 3 or Division 1 Nurse with Psychiatric Endorsement from the Nurses Board of Victoria.

  • Current registration or eligibility for appropriate professional body.

  • Registration with Australian Health Practitioner Regulation Agency with the exception of Social Work.

Specific Training Course:

  • Post graduate qualifications in a relevant area of specialty or in the process of achieving.
  • A current Victorian driver's licence.
  • A current police check

Knowledge & Experience:

  • A minimum of 5 years post qualification in psychology, a minimum of 5 years post qualification in nursing, and a minimum of 7 years post qualification in Occupational Therapy or Social Work. 
OTHER ESSENTIAL REQUIREMENTS
  • Proven ability to provide high quality mental health interventions to people with serious mental illness, including assessment, treatment, case management, crisis intervention, rehabilitation, demonstrated skills in engaging consumers with complex needs and a capacity to identify and respond to dual diagnosis clients
  • Extensive experience in the delivery of clinical mental health services and preferably in a diverse range of mental health service settings, knowledge of relevant legislation, mental health policy directions and principles and practice of community based service delivery including case management.
  • Proven ability and commitment to working collaboratively with consumers, carers, members of the multidisciplinary team, other senior staff and external agencies.
  • Commitment to ongoing professional development and professional supervision.
  • Well-developed interpersonal and communication skills, including sound oral and written communication skills.
  • Well-developed understanding of quality improvement and evidence of implementation and evaluation of quality improvement activities.
  • Commitment to work in accordance with the values of the Sisters of Charity
  • Sound working knowledge of the Victorian Mental Health Act 2014 and other relevant legislation.
