About the role

We are seeking a passionate and motivated Grade 2 Physiotherapist to join our Western Health at Home Division. In this rewarding community-based role, you will provide physiotherapy assessment and management to patients referred to services such as the Community Based Transition Care Program (TCP), Rapid Allied Health (RAH), and other Western Health at Home programs.

This position will offer the opportunity to work across a variety of services within Western Health at Home, supporting patients to achieve their goals and maximise independence in their own homes and communities. You'll be part of a collaborative, multidisciplinary team committed to delivering high-quality, patient-centred care.

The role involves providing physiotherapy assessment and management in patients' homes, in clinic, or via Telehealth to support safe discharge, prevent complications, and reduce hospitalisation. The caseload will be varied and may include musculoskeletal conditions, age-related issues such as falls, neurological and vestibular conditions, and other complex presentations.

About you:

• Be a qualified Physiotherapist with current or eligible AHPRA registration
• Have a minimum of 2 years' clinical experience in relevant acute, subacute, and/or community settings
• Demonstrate excellent communication and time management skills
• Be a collaborative team player who also enjoys working autonomously in patients' homes
• Have a strong commitment to delivering high-quality care to patients in their own homes
• Hold a current Victorian Driver's Licence
