On 1st January 2026, five health services (Kooweerup Regional Health Service, Peninsula Health, Alfred Health, Bass Coast Health and Gippsland Southern Health Service) will come together to create Bayside Health; a newly merged health service with more than 22,000 employees caring for a community of over 1.2 million Victorians.
The new health service will provide people with the best possible care with services that support them throughout their lives. The merger will also strengthen our workforce, providing new training, education and career opportunities. Subject to final approval on 1st January 2026, all employees from the five health services will transfer to Bayside Health.
The Team & Role
Peninsula Health are seeking a highly motivated Occupational Therapist, Physiotherapist or Social Worker to lead a new Pilot Program 'Seniors Lifestyle Victoria'. The Team Leader plays a key role in leading program delivery and coordinating patient care. This exciting position involves both clinical responsibilities such as intake, assessment, care planning, and coordination; and non-clinical duties including data collection for pilot program evaluation and acts as the key interface between clients, internal teams, and external services to ensure referrals are timely, and optimised for individual needs. The Team Leader plays a key role in leading program delivery and coordinating patient care. Working within a newly formed team, the Team Leader ensures high-quality, person centred care aligned with Peninsula Health standards and policies. The role also includes leadership in growing and shaping this innovative program.

Qualifications and/or experience
- Current registration with AHPRA
- Current Victorian Driver's licence
- Proven ability to lead effectively within multidisciplinary teams.
- Demonstrated commitment to delivering high quality outcomes for consumers.
- Demonstrated clinical knowledge and skills commensurate with experience
- Motivational Interviewing or Health Coaching experience and understanding
