Superior Home Health Services are seeking a qualified, reliable and passionate Home and Community Enrolled (or Registered) Nurse Care Coordinator to join our team at the North Lakes - Head Office.

Responsibilities will include:

  • Working within a person-centered, enabling environment that respects the rights and needs of each client;
  • Manage case management for a variety of aged and disability clients;
  • Manage Home Care Package funding administrative tasks;
  • Assisting clients with personal hygiene, dressing, and domestic duties;
  • Assisting clients with daily errands and transport where required;
  • Providing support to access social and recreational activities;
  • Assisting clients with medications;
  • Observe, care and report any changes in the health and wellbeing of the client;
  • Work under our RN Clinical Care Advisor to undertake skilled nursing tasks,
  • Responding to emergency situations

Essential Criteria:

  • Minimum Diploma of Nursing
  • Excellent verbal and written communication skills
  • Willingness to learn, develop and take on new challenges
  • Current National Police Check
  • Current First Aid & CPR certificates
  • Current unrestricted driver's license
  • Medication Assistance
  • A reliable, well-maintained vehicle
  • Comprehensive car insurance
  • NDIS Worker Screening including Blue Card (or equivalent)
  • Community experience, 1 year minimum
