UPA Home Care helps older people live independently by providing in-home services that meet their individual needs whilst promoting wellness, choice, and quality of life. We are offering an exciting opportunity for a Care Partner to join our Far North Coast team based in Tweed Headsd.
About the role
The Home Care Partner works in partnership with clients and the care team to co-design care plans based on the client's goals, preferences, and assessed needs. The role supports functional independence, wellness and lifestyles choices. You'll also liaise with internal and external stakeholders to achieve positive client outcomes within resource and budget allocations.
What you bring
- Tertiary Qualification or equivalent experience in Aged care, Community Services or a health related discipline
- Excellent communication and interpersonal skills
- Experience in coordination, case management and care planning
- Knowledge of Aged Care legislation, principles and Home Care Standards
- Competency using programs such as Microsoft Office and electronic client records
- Current Driver Licence
- Willingness to undergo Criminal History Screening (conducted by UPA)
- First Aid Certificate (or willingness to obtain)
