Team Leader – Lead with Purpose and Make a Difference

Location: South Melbourne | Type: Full-Time | Hybrid: Tues–Thurs in-office

Are you a natural leader with a passion for quality care and empowering teams?

We're looking for an experienced Team Leader to guide and support our Support Partners and Registered Nurses in delivering exceptional, person-centred care to older Australians.

Reporting to the Head of Operations, you'll drive compliance, mentor your team, and ensure the highest standards of service and care are maintained — enabling customers to live safely and independently at home.

What You'll Be Doing
  • Lead, support and mentor a team of Support Partners and Registered Nurses to deliver quality, compliant care
  • Provide clinical oversight and guidance, including Telehealth nursing assessments where required
  • Oversee the development and review of Care Plans and Help Plans in collaboration with clients and stakeholders
  • Ensure services meet Home Care Package (HCP) Guidelines and all regulatory responsibilities
  • Monitor team compliance, processes and documentation to ensure quality outcomes
  • Foster a positive, collaborative, high-performing team culture

What You'll Bring
  • Proven experience leading and managing a multidisciplinary team
  • Excellent understanding of Aged Care regulations, compliance and HCP Guidelines
  • Strong administrative and computer skills
  • Clinical qualification (Registered Nurse, Div 1) highly regarded
  • Experience in Care Management within Aged Care or Home Care
  • Strong leadership, mentoring and communication skills
