Ram Care is Aged Care Provider, dedicated to caring for you and your family.

We are seeking for experienced Homecare package case coordinator to start with us ASAP. We are a small team in springwood office and happy to share that we are growing each year. The hours are Monday to Friday 8:30-4:30pm.

ABOUT THE ORGANISATION

We provide services from: Support Participants with My Aged Care Registration / Support / Navigation, Aged Care Specialist Adviser support, Arrange ACAT Assessments, Helping Patients with Home Care Packages, Organising Emergency in home care, Emergency Equipment Hires and NDIS.

ABOUT THE ROLE

Become part of a dedicated and passionate team, work alongside with Case manager, other Case Coordinators and Roster officer to ensure effective delivery of services. Your days will be filled with wonderful experiences as you meet with our clients, get to know them and their preferences, coordinate their care and support needs, achieving the very best practice care and support for them.

Job Responsibilities:

  • High level experience in the Home and Community Care Sector as a Care Partner
  • Strong understanding of Home Care Package Funding and Service Models
  • Strong capability to effectively coordinate participant care and support services
  • Calm and Caring with Excellence in Participant customer service
  • Exceptional organisation skills
  • Able to commit to continuous improvement
  • Ability to work independently and also as a team ( team player)
  • Demonstrated Empathy, Understanding, and Experience of working with vulnerable people living with Dementia.

  • Understanding of wellness and enablement principles and how they apply to the delivery of care services
  • Experience with challenging behaviours
  • Practical understanding of Hygiene assistance and commitment of care
  • Reliable and emotional stability
  • Strong communication and organisation skills

  • Attention to detail and problem-solving skills

Qualifications/Experiences

  • Previous care coordination experience - minimum 1-2 years
  • Relevant qualifications or experience in leading direct care staff.
  • Computer proficiency
  • must have organisational and time management skills
  • Professionalism
  • National Police Check- Current within 6 months or willing to obtain
  • Blue Card and NDIS Worker Screening or willing to obtain
