Seeking an experienced Disability Support Facilitator

Our client is seeking a Disability Support Facilitator to join their team. They're a trusted NDIS provider based on the Gold Coast. The team delivers personalised support tailored to everyone's lifestyle, goals, and aspirations. Their mission is to empower and respect every person they serve, fostering a community where individuals are supported to thrive and achieve independence.

Your new role

As a Disability Support Facilitator, your key responsibilities will include:

  • Deliver comprehensive case management support to NDIS participants.
  • Facilitate referrals and coordinate care within a multidisciplinary team.
  • Manage a caseload through in-home service delivery.
  • Ensure service quality exceeds accreditation standards.
  • Accurate, timely documentation and progress reporting.
  • Provide effective leadership and supervision to team members
  • Oversee staff rostering and allocation in line with the Collective Agreement.
  • Liaise regularly with participants and their primary carers to ensure continuity of care

What you'll need to succeed
- You will be both professional and compassionate
- At least 2-3 years in a similar role with experience in working with participants with complex medical needs
- Obtained NDIS screening card
- Great attention to detail, exceptional phone manner and email correspondents
- Work collaboratively in a team
- Proficient in Microsoft Suite and bonus if you have experience with Shiftcare
