About the Role

An opportunity currently exists for an experienced Grade 2 Social Worker to join our team on a permanent basis. At St Vincent's we have an approach to working with patients which puts them at the heart of their care. This position will work with patients admitted to St Vincent's Hospital, Fitzroy. To be successful in this position, it is essential that you have experience working in an inpatient hospital social work position.

You will be required to:

• Use your skills to undertake multi-dimensional, comprehensive psychosocial assessments, apply clinical reasoning, and formulate appropriate complex intervention and discharge plans
• Liaise with patients, their families and/or carers to co-ordinate, facilitate and communicate patient treatment and outcomes
• Actively participate in quality improvement and research activities
• Provide supervision and participate in clinical education programs as required
• Promote and develop positive relationships within the department, wider organisation and with community partners

Classifications for Grade 2 Social Workers range from SC21 - SC24 ($90,261 - $103,724 per annum).

Your Contribution

• Bachelor Degree or Qualifying Masters in Social Work
• Significant social work experience in an Australian hospital setting is essential
• Current National criminal history check, or willing to obtain
• Eligible for membership of the Australian Association of Social Workers
