This vital role is dedicated to providing responsive, patient-centered support to brain cancer patients diagnosed with a primary brain tumour in the Canberra region. Acting as a bridge between patients, families, healthcare providers, and service networks, you will ensure the highest quality support and guidance is delivered at every stage of the brain cancer journey—from diagnosis to bereavement.

Key Responsibilities include:

Patient and family support: Offer personalised information and care plans, empowering patients and families to actively participate in treatment decisions.

Home visits: Provide compassionate in-home support, including informal counselling, companionship, and practical assistance.

Education and empowerment: Simplify complex medical information, helping patients and families understand diagnoses, treatment options, and available resources.

Advance care planning: Assist patients and families with creating and finalising care plans, preparing them for future needs.

Bereavement support: Offer grounded, compassionate presence through end-of-life experiences, including support for death, funerals, and grief.

Referral coordination: Facilitate access to brain cancer-specific counselling, legal services, and pro bono resources.

The position is a joint collaborative between Peace of Mind Foundation, who manage and deliver the Community Brain Cancer Navigator program, and Australian Brain Cancer Foundation (ABCF). This role is made possible through the ABCF's Brain Cancer Support Team initiative and funded by its 13 Founding Partners. The role will be known in the Canberra community as the Canberra Brain Cancer Support Team.

To excel in this role, you should possess:

A relevant tertiary qualification in a health/welfare related discipline

Demonstrated experience in and/or commitment to working with people living with a life limiting disease

Demonstrated experience and knowledge of support and care planning in the disability services or cancer services arena

Highly developed interpersonal communication skills, both verbal and written

Demonstrated experience working independently and as part of a team

Demonstrated negotiation and creative problem-solving skills

Demonstrated ability to manage competing work tasks and work effectively to deadlines

Demonstrated computer literacy and understanding of electronic databases

Demonstrated ability to develop relationships with service providers and identify appropriate services to meet client needs

Current ACT Driver's Licence
