About the role

  • Case Management of clients who have sustained significantly either on NSW roads or at a workplace.
  • Discipline specific assessment and or service (for example vocational /avocational planning etc)
  • Attending medical appointments and / or team meetings relevant to client needs

  • Liaising with stakeholders

  • Travelling to see clients in their homes and/or community

  • Encouraging and planning independence

  • Report writing (person centred planning / service requests)

  • Communicating the needs of participants and ensuring they receive the appropriate support and resources

  • Maintaining accurate and detailed case records in line with legislative and organisational requirements

About you

  • Allied Health Qualifications in Occupational Therapy, Rehabilitation Counselling, Psychology, Nursing, Social Work, Physiotherapy or Exercise Physiology
  • Professional membership with relevant body and / or AHPRA

  • Minimum 5 years working in a similar role (working with significant injury including traumatic brain injury, amputation, spinal cord injury)

  • Strong problem-solving and decision-making skills, with the ability to think creatively

  • Experience working with technology including customer data bases / email etc
  • Excellent communication and interpersonal skills, with a compassionate and empathetic approach
  • Proficient in case management principles, rehabilitation processes and relevant legislation
  • Demonstrated ability to work under a KPI model
  • Working rights in Australia
