About Us?The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region.
The focus of IUIH Aged Care team is to work with the local Indigenous community and relevant key stakeholders to increase access to comprehensive care and provide well-coordinated, quality in home care services to eligible aged care clients.
About the Role?This position plays a key role within an innovative pilot program delivering culturally safe, person-centred aged care Assessment services to older Aboriginal and Torres Strait Islander people, operating in one or both Caboolture and Sunshine Coast Aged Care Planning Regions (ACPRs). The position combines the responsibilities for the Triage process for clients referred by the Team Leader as well as the responsibilities to approve a person as eligible to receive different types of aged care as a "delegate" of the Secretary of the Department of Health, Disabilities and Ageing. You will be required to undertake culturally appropriate aged care Comprehensive Assessments, and occasionally Home Support Assessments of eligible Aboriginal and Torres Strait older persons.
We have a full-time temporary role available. Within this role, you will be required to:
- Be the initial contact with an older person and their support people by performing an initial needs assessment as part of the triage process.
- Utilise your clinical judgement to determine the assessment type and the priority/urgency in which care can be provided.
- Undertake the triage process to confirm eligibility for an assessment
- Update client information in My Aged Care to keep records accurate and up to date.
- Undertake assessments as allocated by the Team Leader, using the Integrated Assessment Tool (IAT), of older Aboriginal and Torres Strait Islander people which do not require clinical judgement, in line with My Aged Care and IUIH procedures
- Refer clients to appropriate services, assess complexity and vulnerability, and assist with linking support, reablement, and short-term case management
About You?
- Current C Class Drivers Licence (QLD
- Current Criminal History Check - required prior to your commencement
- Current Disability Workers Screening Card (QLD Yellow Card) - required prior to your commencement
- Tertiary qualification in a health-related discipline directly related to health, aged care or related specialist area.
- Current unrestricted APHRA Registration
- Minimum of one year demonstrated ACAT assessment experience in Australia.
Desirable, but not mandatory:
- Knowledge of Aboriginal and Torres Strait Island culture through either identifying as Aboriginal or Torres Strait Island person and/or demonstrated experience of working successfully within Aboriginal and Torres Strait Island communities.
- Knowledge of current Australian Aged Care System reforms and access, assessment, service delivery arrangements.
