Registered Nurse / Case Manager

We're currently seeking a Registered Nurse / Case Manager to join our team based in Camp Hill. This role is ideal for a proactive and compassionate leader who thrives in a collaborative environment and is committed to delivering outstanding care outcomes.

Key Responsibilities

  • Deliver holistic case management from initial referral through to service planning, implementation, and ongoing reviews
  • Develop and manage individual care plans and budgets in consultation with participants, carers, and families
  • Ensure services are implemented in line with client care plans and within NDIS and CDC frameworks
  • Conduct clinical reviews and provide clinical advice and training to support staff
  • Build strong, respectful relationships with participants, families, and support networks
  • Help participants set and achieve meaningful, realistic goals
  • Travel to clients across Brisbane, Logan, and surrounding areas as needed

About You

To be successful in this role, you'll bring:

  • A nursing qualification and current AHPRA registration
  • Strong clinical knowledge and problem-solving ability
  • Experience and knowledge in the NDIS and My Aged Care (Home Care Packages) sector highly favourable
  • Excellent communication and customer service skills
  • Ability to build positive relationships with participants, families, and team members
  • A current, unrestricted Driver's Licence and reliable vehicle

Applicants with NDIS sector experience will be highly regarded.
