About us:

The Integrated Chronic Care for Aboriginal People Program (ICCAPP) is a community outreach service that aims to increase access to culturally appropriate health services for Aboriginal and Torres Strait Islander people aged 15 years and older, who are at risk of or who have a chronic disease.

We have an excellent opportunity for a motivated organised professional to become a member of the Integrated Chronic Care for Aboriginal People Program (ICCAP).

This position is based at Cessnock District Hospital Campus, providing services to Aboriginal people who reside in the LGA and communities that are served by the Lower/Upper Hunter Sectors. The Lower/Upper Hunter Sector ICCAP team consists of a Clinical Nurse Consultant and an Aboriginal Health Practitioner and is managed by the Nurse Manager Cessnock Community Health Service. Our unit focuses on maintaining the health and wellness of Aboriginal people in community including those with chronic disease and those recently discharged form hospital. The program aims to provide a practical approach to improving health outcomes for Aboriginal people with or at risk of developing a chronic disease by working in partnership, to develop and support flexible local approaches to chronic disease.

About the role:

As an Aboriginal Health Practitioner / Aboriginal Health Worker you will work in conjunction with the Clinical Nurse Consultant ICCAPP, and in collaboration with both internal and external partners (clinical and non-clinical) to deliver flexible, holistic, culturally sensitive services for Aboriginal clients, including liaison with relevant community groups, health professionals, and partner organisations. This role involves clinical assessment, screening, case management, education and referral for Aboriginal and Torres Strait Islander people accessing the ICCAPP service.

Our successful applicant will be supported in this role by the Aboriginal Health Practitioner and ICCAP Clinical Nurse Consultant. Our successful applicant will be working in a small team and be supported in this role by both the ICCAP Clinical Nurse Consultant locally as well as the wider ICCAP team.

About you:

We're looking for a motivated and organised professional with a strong understanding of Aboriginal and Torres Strait Islander cultures. The ideal candidate will be confident in building respectful relationships and communicating effectively with Aboriginal and Torres Strait Islander individuals, families, and communities. Your cultural knowledge and ability to work collaboratively will be key to supporting culturally safe and responsive care.

Requirements:

  • Holds a Certificate IV in Aboriginal Primary Health Care (Practice) and a current authority to practice as an Aboriginal Health Practitioner with the Australian Health Practitioner Regulation Agency (AHPRA) OR holds a minimum certificate III qualification in Aboriginal Primary Health care or a minimum Certificate III health qualification in the area of care in which the AHW works AND be willing to undertake a Certificate IV within 18 months to enable transition to Aboriginal Health Practitioner status with current AHPRA registration.
  • Eligible to drive in NSW and the ability to travel as required.
