  • Provide in-patients and their families and carers with high quality, person-centred support during their stay;
  • Plan and coordinate appropriate services for discharge in conjunction with multidisciplinary team;
  • Work towards quality improvements in social work and multidisciplinary services in line with War Memorial Hospital's overall objectives.
Selection Criteria:
  • Minimum of 2 years' experience working as a Social Worker and a demonstrated knowledge and expertise in Social Work practice in an aged care and/or community setting
  • Demonstrated ability to provide effective Social Work services to older people, including skills in psychosocial assessment and management, comprehensive client centered assessment and care plan development outlining clinical reasoning from chronic and complex health care needs clients
  • Demonstrated ability to provide case management in consultation with the client, carer and multidisciplinary team
  • Demonstrated knowledge of relevant community services, professional networks and residential aged care services for clients with complex aged related psychosocial issues and/or chronic complex health care needs
  • Ability to work effectively both autonomously and within a multidisciplinary team and to exercise independent professional judgement on routine matters, commensurate with years of experience
  • Demonstrated high level written and verbal communication, and interpersonal skills, and the ability to proactively engage with patients/clients across all levels and departments within WMH and external stakeholders
  • Current NSW Drivers' license Class C
  • Degree or postgraduate qualification in Social Work and eligibility for registration with the Australian Association of Social Workers (AASW)
