About The Role
Are you a dynamic healthcare professional (nursing or allied health) with a passion for improving access to specialist care for older adults? Western Health is offering an exciting opportunity to join our team as the Coordinator of Geriatric Medicine Clinics.
In this pivotal role, you will lead the efficient delivery of high-quality, patient-centred Geriatric Medicine Clinics. Your expertise will support timely access to specialist geriatric assessment and management, helping to improve outcomes and quality of life for older adults in our community.
Key Responsibilities
- Lead and coordinate the daily operational and clinical functions of the Geriatric Medicine Clinics
- Manage internal and external referrals to ensure a streamlined and responsive patient journey
- Apply clinical knowledge to triage and schedule referrals in collaboration with medical and administrative staff
- Engage directly with patients to support referral triage, connect them with primary care, and facilitate ongoing referrals to support services
About You
- Have appropriately recognised tertiary qualification and registration with the Australian Health Practitioner Regulation Agency (AHPRA) eg. Registered Nurse, Allied Health Professional
- Proven clinical expertise, ideally gained in public health environments and in delivering care to older populations
- A commitment to delivering patient-centred care, with a deep understanding of the unique needs of older adults
- Strong clinical assessment, triage, and coordination skills
- Demonstrated leadership capabilities and a passion for continuous service improvement
- Excellent communication, interpersonal, and stakeholder management skills
- Strong organisational and problem-solving abilities
- A commitment to delivering safe, high-quality care aligned with Western Health's values and the Live Best Care framework
- Proficiency in electronic medical records and referral management systems
