Redeema has a rewarding opportunity for a Clinical Nurse Consultant- NDIS Lead with a passion for working with adults in the NDIS who have High Physical Support needs, Complex and High Intensity Supports. This is a part-time position (4 days initially) pro-rata at $115,000-$120,000 plus 11.5% Superannuation. The incumbent will have the choice to scale-up to a full-time role as the demand for services increase. This key position will also provide Health Support Training to our internal staff, manage a small caseload and deliver on Training for Heath Support to the wider sector. The position is based in our head office at Port Melbourne and flexible/hybrid working arrangements are available by negotiation. The position directly reports to Redeema's Chief Executive Officer.

About the role

  • Key role in the establishment of a CNC led clinic model and the provision of high-quality healthcare assessment, planning and coordination for NDIS Participants with a key focus for Participants with High Intensity Support or High Physical care.
  • The CNC will work closely with the clinic team, including the medical practitioner and allied health clinicians as required, as well as working independently to coordinate the service and complete health care plans.
  • Coordination and management of referrals and the client clinical pathways
  • Providing timely and comprehensive written reports/health care plans either independently or in conjunction with the clinic team
  • Completion of professional development/education, reporting and evaluation activities of the service
  • Deliver competency based training to field staff as needed and tailor to the participants Health Support Plan NDIS Goals and funding outcomes
  • Manage a small case load initially while we grow the Training program state-wide with the view to increase staff FTE
  • Deliver Health Support Training to the wider sector requesting tailored training to their NDIS DSWs or Aged Care Staff
  • Work alongside Redeema's Management Team to pitch for high intensity participants
  • Provide clinical justification to support hospital allied health team as part of their pre-planning tool or NDIS plan reviews

Essential Criteria

  • Tertiary qualification in Nursing & minimum of 5 years full time post qualification experience (compulsory)
  • Current registration as a Registered Nursing with the Nursing & Midwifery Board of Australia (AHPRA) (compulsory)
  • Certificate IV in Training Assessment  (compulsory)
  • NDIS Worker Screening Clearance (or willingness to obtain)
  • Knowledge of the NDIS and the disability sector, including NDIS Quality & Safeguards Framework
  • Comprehensive understanding of clinical governance in a disability setting (compulsory)
  • Experience in providing assessment and consultation for adults with intellectual disability and complex medical, physical and or mental health/ behavioral conditions.
  • Demonstrate excellent interpersonal and communication skills with clinic staff, external agencies, people with intellectual disability and their families/carers
  • Excellent written communication skills, with a high level of attention to detail to produce succinct high-quality reports
  • Demonstrated administrative and advanced MS Office Skills
