About the role

• The Chronic and Complex Care (CCC) Service provides specialised, team-based care for people living with long-term or complicated health conditions who are starting to need more support.

• CCC is a short-term but high-impact service that acts as a bridge between hospital care and regular community care.

• The CCC Service provides flexible and patient-centred care through various delivery modes, tailored to meet clinical needs and patient preferences: home visits, telehealth, multidisciplinary clinics and group classes.

• Providing specialist nursing services to ambulatory and community-based clients, to fulfil the strategic and operational plans of Subacute Care Services and St Vincent's Hospital.

• Responsible for the provision of clinical support, wound assessment and quality nursing care to clients based on practice principles.

About you

• Currently registered as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency.

• Relevant post-registration qualifications and at least three years' experience working in the clinical area of their specified post-graduate qualification.

• Current unrestricted NSW Driver's License.

• Demonstrated commitment to ongoing professional development and demonstrated effective communication and interpersonal skills.

• Demonstrated evidence of integration of theoretical knowledge and clinical skills, critical thinking and analysis in their daily practice as a registered nurse.

• Demonstrated effective time management skills and ability to prioritise competing workloads within a complex environment.
