About the role
The Chronic and Complex Care (CCC) Service provides specialised, team-based care for people living with long-term or complicated health conditions who are starting to need more support.
CCC is a short-term but high-impact service that acts as a bridge between hospital care and regular community care.
The CCC Service provides flexible and patient-centred care through various delivery modes, tailored to meet clinical needs and patient preferences: home visits, telehealth, multidisciplinary clinics and group classes.
Providing specialist nursing services to ambulatory and community-based clients, to fulfil the strategic and operational plans of Subacute Care Services and St Vincent's Hospital.
Responsible for the provision of clinical support, wound assessment and quality nursing care to clients based on practice principles.
About you
Currently registered as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency.
Relevant post-registration qualifications and at least three years' experience working in the clinical area of their specified post-graduate qualification.
Current unrestricted NSW Driver's License.
Demonstrated commitment to ongoing professional development and demonstrated effective communication and interpersonal skills.
Demonstrated evidence of integration of theoretical knowledge and clinical skills, critical thinking and analysis in their daily practice as a registered nurse.
Demonstrated effective time management skills and ability to prioritise competing workloads within a complex environment.
