About the Role
As a Clinical Pharmacist at Slade Pharmacy Port Macquarie, you'll deliver clinical pharmacy services within Port Macquarie Base Hospital, a major regional facility offering a wide range of medical services. You'll work closely with doctors, nurses, and allied health professionals to support patient care.
Responsibilities
- Deliver comprehensive clinical pharmacy services in a hospital setting.
- Conduct medication history interviews and reconciliation at transitions of care.
- Provide medication counselling and education to patients.
- Monitor medication outcomes and manage adverse drug reactions.
- Collaborate with the healthcare team to ensure optimal patient outcomes.
- Participate in training and quality improvement initiatives.
- Monday to Friday with rotational on-call and weekend shifts. Penalty rates and allowances applicable.
About You

Essential Requirements
- Current AHPRA general registration as a Pharmacist.
- Australian working rights for a minimum duration of 12-months.
