Nurse Practitioner / Transitional Nurse Practitioner – Diabetes Service

You will be working alongside a team that provides inpatient and outpatient diabetes care for people of all ages with Type 1 and Type 2 diabetes, and diabetes in pregnancy.

We're looking for a passionate Nurse Practitioner (NP) or Transitional Nurse Practitioner (TNP) to provide expert diabetes care in a fast-paced, community-focused environment where your leadership, clinical expertise, and compassion will truly shine.

Where You'll Be Working

Based at the Tamworth Community Health Service, located within Tamworth Hospital, you'll work alongside a small, dedicated team of Registered Nurses and a Dietitian. Together, you'll deliver comprehensive, patient-centred diabetes care across both acute and community settings.

What You'll Be Doing

As a Nurse Practitioner in Diabetes, you'll deliver advanced clinical care using your highly developed nursing knowledge and skills.

Your responsibilities will include:

  • Conducting comprehensive assessments and managing patients with diabetes
  • Initiating and interpreting diagnostic investigations
  • Prescribing medications within your scope of practice
  • Referring patients directly to other healthcare professionals as required
  • Enhancing the interface between acute and primary care services through collaboration and communication with multidisciplinary teams
  • You'll provide care to people of all ages across acute, outpatient, and community settings, making a real difference in diabetes management and prevention

Requirements

  • Current endorsement as a Diabetes Nurse Practitioner, or working toward endorsement (Transitional Nurse Practitioners welcome)
  • Current AHPRA registration as a Registered Nurse
  • Minimum 3 years full-time advanced practice experience in diabetes (within the past 6 years)
  • Postgraduate qualifications at master's level (or currently completing) in an NMBA-approved program
