About the Role:

As a Toward Home Specialist Outreach Worker, you'll play a key role in delivering health-focused outreach support to people experiencing homelessness with complex needs. Working across the inner city and southern suburbs, you'll provide mobile assessments, brief interventions, and connect clients to essential health and community services. You'll manage a small caseload and collaborate closely with the Toward Home Resolve team to deliver coordinated, person-led care.

In this role, you'll draw on your proven ability to conduct mental health and AOD assessments, deliver psychoeducation to vulnerable and at-risk populations, and communicate effectively with clients, partner agencies, and the broader community. You'll work collaboratively within a multidisciplinary team while maintaining strong relationships with health professionals and support staff. Success in this role requires motivation, initiative, flexibility, and the ability to work both independently and as part of a dynamic team. Strong IT skills and a commitment to providing high-quality, person-centred care are essential.

Experience Requirements:

  • Minimum 2 years' experience in relevant field – AOD/Mental Health
  • Minimum 2 years' experience working with vulnerable individuals.
  • Experience working with people with high and complex health needs

Qualification and Other Requirements:

  • Relevant qualifications such as Social Work or Occupational Therapy
  • Current or willing to obtain a Working with Children's Check (WWCC)
  • Current or willing to obtain a National Police Check (NPC)
