About the Role:

The Clare Holland House, Social Work team at North Canberra is responsible for the care and support of patients accessing Specialist Palliative Care.

Social workers provide assessment and therapeutic intervention for patients throughout their Palliative Care journey. The patients we see present with a range of psychosocial issues that impact their health during their end-of-life care.

The Social Worker will understand issues related to health and wellbeing and the impact on the person and their family/carer, including adjustment to change in their health. The Social Worker will promote improved client outcomes through working in collaboration with the multidisciplinary team to provide high quality Specialist Palliative Care services.

  • Undergo a pre-employment National Police Check.
  • Comply with Canberra Health Services Occupational Assessment, Screening and Vaccination policy.
  • Comply with CHS credentialing and scope of clinical practice requirements for allied health professionals.

Notes: A merit pool may be established from this recruitment process and will be used for the next 12 months to fill various positions both on a temporary and permanent basis with full-time and part-time opportunities available.
