The Job

The Community Nurse (RN) will collaborate with care partners and other healthcare professionals to deliver high-quality nursing care to customers in their homes. The Community Nurse will assess customer's health needs, develop nursing care plans and provide skilled nursing interventions to promote health and wellbeing of our aged care and NDIS customers. The position is available on either a part time or full time basis.

Key Outcomes

Clinical Care

  • Provide direct clinical care services to Lifebridge customers with clinical outcomes in line with best practice standards for care delivery.
  • Schedule and conduct health assessments and ongoing evaluations of customer's health needs including physical, psychological and social aspects.
  • Use specialist clinical knowledge to assist the management of customers with complex needs including the development of goals, assessed needs, care plan and objectives.
  • Develop individualised nursing care plans in collaboration with care partners, customers, their families, and other healthcare professionals.
  • Provide skilled nursing interventions, treatments, and procedures as prescribed, including wound care, medication management, symptom management and palliative care support and monitor customers' progress and response to treatment.
  • Advocate for customers' rights and access to appropriate healthcare services and resources.
  • Maintain regular contact with customers and their family/carers to ensure that services are beneficial, their needs are still being met and clinical goals achieved.
  • Participate in case conferences and team meetings to discuss client care plans, progress, discharge planning and clinical supports.
  • Maintain accurate and up to date documentation of assessments, nursing care plans, interventions, correspondence with allied health providers and customer interactions.

Compliance:

  • Maintain valid and up to date Nursing qualifications and APHRA registration.
  • Comply with Aged Care and NDIS Quality and Safeguarding Standards, program guidelines, Lifebridge policies and procedures and the ethical and legal standards of nursing practice.
  • Prepare for, conduct, and report clinical audits as required.

Training & Development:

  • Provide clinical related education to Lifebridge staff e.g. Medication Support, Infection Control.
  • Educate customers, carers and families on health promotion, disease prevention, and self-management strategies to support wellness and enablement.
  • Participate in quality improvement initiatives and professional development activities to enhance nursing practice and ensure compliance with regulatory standards.

Knowledge / Skills Required

  1. Registered Nurse with current APRHA registration.
  2. Clinical nursing experience (5+ years), preferably in the community, home, residential health settings.
  3. Well developed clinical knowledge and commitment to excellence in nursing practice and applications.
  4. Knowledge of current care management practices for the frailer aged, people living with dementia and people with complex care needs.
  5. Knowledge of Aged Care and NDIS regulatory and legislative frameworks and requirements.
  6. Exceptional customer service, communication and relationship management skills.
  7. Ability to work independently and collaboratively within a multidisciplinary team.
  8. High level of organisation and time management skills, including the ability to reorganise work priorities to meet changing demands.
  9. Excellent computer skills and ability to utilize customer management systems.
  10. Current drivers licence and roadworthy vehicle.
