These services are to be least restrictive, collaborative, nominated carers and family responsive and inclusive, rehabilitative, encourages self-advocacy, facilitates empowerment and capacity for optimal independent function.

A primary goal of this position is to work collaboratively with existing inpatient and community teams, consumers, carers and other key stakeholders in the coordination of day-to-day work practices for teams working with PCLI Consumers, by supporting the assessment and progression of safe transitions of consumers into care in a community setting while promoting effective participation in mental health treatment, support and recovery.
