Pathways to Community Living Initiative (PCLI) is a coordinated state-wide approach to supporting people with enduring and serious mental illness who have been in hospital for more than twelve months and or are likely to experience difficulty with discharge solutions, to re-establish their lives in the community.

The role of the Senior Clinician is to provide district-level evidence-based care that supports safe transitions of consumers into care in a community setting (or appropriate inpatient setting) based on the individual consumer's strengths, needs and wishes. These services are to be least restrictive, collaborative, nominated carers and family responsive and inclusive, rehabilitative, encourages self-advocacy, facilitates empowerment and capacity for optimal independent function.
A primary goal of this position is to work collaboratively with existing inpatient and community teams, consumers, carers and other key stakeholders in the coordination of day-to-day work practices for teams working with PCLI Consumers, by supporting the assessment and progression of safe transitions of consumers into care in a community setting while promoting effective participation in mental health treatment, support and recovery.
