UMFC is seeking compassionate and skilled Interchange Practitioners to support children and people with disability through person centred and family-focused programs. This position is offered on a full-time basis.

About the Role

The Interchange Practitioner assesses, plans and delivers tailored individual and group programs to children and people with disability, supporting their personal goals and aspirations. You will coordinate and guide Flexible Support Workers to ensure programs are delivered effectively and meet client needs. Reporting to the CaDS Senior Team Leader and Service Manager, you will also manage all intake, service delivery and case closure tasks in accordance with organisational policies and procedures.

About You

We are looking for someone who is passionate, skilled, and ready to make a difference. You will bring:


• Diploma or Degree in Community Services, Social Work, Human Services, or a related field with relevant experience.


• Extensive experience in community services with family-centred and person-centred approaches.


• Skilled in leading tailored programs, maintaining detailed records, and ensuring compliance.


• Strong interpersonal, communication, and emotional intelligence skills.


• Knowledge of duty of care, OHS, and risk management.
