We're entering an exciting new chapter. Under the leadership of our Chief Services Officer and Community Supports Manager, we're reimagining how disability services are coordinated, delivered, and experienced by our team. About the Opportunity:
As a Community Supports Coordinator, you'll be at the centre of our new service coordination model – supporting people to live with greater choice, independence, and connection to their community. You'll coordinate supports, lead and work alongside a great team, and build strong partnerships that put individuals and their goals first.
This isn't just about keeping things running smoothly (though you'll do that too). It's about creating possibilities, fostering inclusion, and helping individuals and their families feel truly supported.
Duties include service planning, participant support, team leadership and performance management, quality assurance, and stakeholder engagement.
What You Will Bring to Our Team:
- Strong value-based service delivery experience to ensure client-focused quality support, along with a dedication and commitment to achieving the best outcome for team and clients alike.
- Experience in establishing and monitoring the delivery of support services for people with disabilities, with an intuitive lens for consistent improvement.
- Experience leading and managing a team in a complex or fast-paced environment, with a focus on employee experience, professional development, mentorship and supervision.
- Knowledge and understanding of the NDIS with a dedication to complying with all industry requirements, including confidentiality, compliance and quality improvement expectations.
- Ability to balance day-to-day service coordination with practice excellence and a forward-thinking approach.
- Strong understanding of and technical skills relating to incident reporting, compliance and management.
- Ability to build strong relationships with team members, clients, families, and partners.
To be Successful You Will Demonstrate:
- The ability to monitor and respond to all changes in client requirements, including the ability to develop individualised strategies, in a timely manner with limited supervision and according to agreed expectations.
- Strong computer skills at an intermediate-advanced level, allowing for efficient administration and documentation processes.
- An ability to be flexible and adaptable; you will be required to respond to challenging situations as they arise to ensure continued and safe services which will require you to undertake on-call duties as part of a rostered system.
- Strong interpersonal and customer relationship management skills, with experience navigating (at times) challenging personalities.
- Professional and supportive communication skills (both written and verbal), with experience communicating during high-stress or complex situations in a calm and measured way.
- Current First Aid and CPR certificate.
- Cert IV or above in Disability / Community services or related field.
- Experience using Carelink software (desirable but not essential).
