About the Role

To provide safe, compassionate and high-quality person-centred assessment and case coordination to clients. Ensuring care is delivered in collaboration with clients, their families/carers and a multidisciplinary team to achieve identified goals and outcomes in line with MNCLHD Aged Care Assessment Service guidelines (ACAS).
