
Care Partner – Tweed Heads

We are seeking a proactive, compassionate, and forward-thinking Care Partner to join our team and play a vital role in delivering high-quality, person-centred care across the Northern Rivers region. With the rollout of reforms under the Support at Home Program, this role offers the opportunity to step beyond traditional case management and into leadership in action.
You will be working from our Tweed Heads office and have use of a company vehicle for all client related travel.
About the Role
As a Care Coordinator, you will be responsible for coordinating care services under the Support at Home framework, ensuring clients are empowered to make informed choices and maintain independence. This includes overseeing services such as allied health, home support, and equipment.
Key Responsibilities:
- Deliver person-centred case management tailored to each client's goals and needs
- Prepare and review budgets with clients to align with their care objectives
- Manage a caseload, conducting regular assessments and reviews
- Build and maintain strong, trusting relationships with clients and families
- Coordinate services across allied health, nursing, and home support
About You
You will thrive in this role if you are a natural coordinator with strong problem-solving and advocacy skills, and a genuine passion for improving the lives of older Australians.
Essential Skills & Experience:
- Previous experience in aged care case management (HCP/CHSP preferred)
- Background in community or aged care service coordination
- Excellent planning, communication, and interpersonal skills
- Ability to work independently while contributing to a multidisciplinary team
- A valid Australian driver's licence and clean driving record
- Eligibility to obtain a National Police Check and NDIS Worker Screening Check
Highly Regarded:
- Nursing qualifications (RN or EN)
