About the roleIn this role, you will provide additional Site-Specific Clinical Funding support to the homes for the Australian National Aged Care Classifications (AN- ACC classifications) with the support of the SCCQ Clinical Funding manager.

Working in this role your responsibilities will include:

  • Liaising with carers and other Clinical employees to ensure they accurately document resident care needs and time spent, which support funding classifications and appropriate staffing reflective of Resident's care needs.
  • Assist and support staff with ensuring they poses resources and knowledge of the systems and raise any concerns with the Clinical Manager.
  • Assist in upskilling Residential Aged Care staff in their knowledge of the funding process by providing training sessions to relevant roles (including but not limited to RN/EN/PCW/Lifestyle Assistants and Pastoral Carers) as required at scheduled sessions agreed by CM/FM
  • Provide ongoing education through toolbox sessions at shift change over to increase awareness using developed toolboxes and incorporate same into the induction program of new employees.
  • Actively participate in monitoring resident conditions, clinical meetings and feedback from clinical teams, handovers and head of department meetings to gather information around potential reclassification opportunities. This includes but is not limited to attending HODS meetings, considering message boards, progress notes, following up handover, reviewing Mirus Metrics/assessment needs, Medicare movement events, information relevant to pending Residents reclassification and liaising with relevant staff to identify changes that may need screening.
  • Assist with the monitoring and tracking of residents' data including all movements, Medicare events and AN-ACC lodgments.

    Active involvement in admission process including but not limited to pre-screening, AN-ACC funding predictions, commencement of assessment and care plans and communication with the clinical

    team.
  • Review assessments to care plan to ensure congruency and ensure it is reflective of residents needs to support AN-ACC Classification. This includes but is not limited to pre-admission screening, new

    admission screening and reclassification screening.
  • Deliver high quality, effective documentation, reporting and tracking. Monitor Occupancy, Entry and Departures.
  • Complete the pre-screening/New Admission/Reclassification forms with all data required to support the forecasted classification and any comments to support.
  • Complete allocated KPI's for resident screening, reclassification requests each week in order to support sites KPI for PRPD.
  • Actively participate in communications with internal stakeholders. This includes but is not limited to attendance at HODS and AN-ACC meetings and sound communication with the Clinical Funding

    Manager
  • Perform (as directed), analysis, data collection and reporting specific for Assessment Management Organisation (AMO) visits to conduct assessments. Ensure AN-ACC assessors have designated areas

    to complete a review, required logins and all relevant information and prepare staff for interviews as required.

What will you bring to the role?

  • Experience in a care or clinical role within a Residential Aged Care Facility
  • Knowledge and understanding of the AN-ACC tools, AN-AC funding guide and processes in a Residential Aged Care Facility
  • Proven ability to quickly acquire knowledge of diverse software systems together with a working knowledge of the Microsoft suite of products
  • Ability to work as part of a team and with minimal supervision
  • Attention to detail and great customer service skills and ethic
  • Strong attention to detail and outstanding communication skills both written and oral
